Initial Nutrition Counseling Session

At your first session we will complete an in depth assessment which includes your medical history, dietary habits, lifestyle, nutritional knowledge, and any other pertinent information. From there, we will work together towards a personalized plan and help set those first achievable goals.

Follow Up Counseling Sessions

At your follow up sessions, we will review your progress. You will receive continued education on various nutritional and wellness topics, and accountability tactics depending on your needs and goals. We will work in further detail towards a more healthy and peaceful relationship with food, love and care for your body, improve energy levels, manage stress, and monitor any medical changes.

  • Insurance

    Proper Nutrition Therapy is In Network with most of the large commercial insurances such as:

    Aetna

    Highmark Blue Cross Blue Shield

    Cigna

    Medicare

    Independence Blue Cross

    United Healthcare (in progress)

    * Most of these insurances have coverage for nutrition counseling.  It is the patients responsibilty to verify their coverage. Please see insurance tab for information of how to verify.

  • Self Pay

    Most of Proper Nutrition Therapy clients have health insurance covering the cost of their appointments. If your insurance does not (or you have exceeded your limit of visits), we do provide prompt-pay and provider discounts (paid before we bill your insurance). We accept cash, check, credit/debit cards, and Health Savings Accounts (HSA) and Flexible Savings Accounts (FSA) to cover your appointment costs.

    *A credit/debit card is required to be put on file.

    Self Pay Rates 60 mins | $125

  • Out Of Network

    Many other health insurance plans also have out-of-network benefits meaning that if we are considered out-of-network with your insurance, sessions may still be covered. Upon request, we can either send a claim to your insurance on your behalf or provide you with a superbill to submit yourself for possible reimbursement. Payment will be for full customary rate and is due from you at the time of service (if we are not in-network with your insurance plan).
